Heart To Hands Home Care is a Home Health Care Agency that provides Personal Care Assistant (PCA) Services to people of all ages in Minnesota. Founded by an RN who is passionate about helping people, and believe in treating our clients and employees like family with integrity, honesty, and respect. Heart to Hands is run by an RN with Home Care experience who wants to bring her skills and knowledge to the client while keeping the client independent in their own home.

Our Staff

Having someone you know and trust as your caregiver is very important. In fact, most long-term care in the U.S. is provided by family members. We understand this and are happy to hire your friends or family members to be your Personal Care Assistants.

Given a choice, most people would rather be cared for at home instead of a hospital or facility. This is possible in Minnesota and people of all ages with special health care needs are living more independently at home with the help of medical and non-medical health related needs.

Our staff includes a PCA of your choice, a Qualified Professional (QP) which is also a Registered Nurse (RN).

What is a PCA?

Personal care assistant is an individual employed by a personal care assistance provider agency, enrolled by DHS and provides personal care assistance services. Personal care assistance (PCA) services provide assistance and support for persons with disabilities living independently in the community. This includes the elderly and others with special health care needs. PCA services are provided in the recipient’s home or in the community when normal life activities take him/her outside the home.

Our PCA employees are required to complete training and pass a background check before providing care to any of our clients. Our friendly office and management staff are highly knowledgeable about the PCA program and other resources for clients and employees.

What is a QP (Qualified Professional)?

The QP is required by law to supervise all PCA services.

The Qualified Professional will assist you with your care plan which is based upon the Public Health Nurse Assessment, assuring that your care plan accurately reflects the Assessment. (Your care plan describes your needs and how the PCA will assist you.)


We also offer private pay skilled nursing services which include-

  • Weekly medication setup and medication management
  • Vital signs - blood pressure, pulse, temperature, heart and lung assessment
  • In-home INR /Coumadin blood draws and reporting to physician
  • In-home injections
  • Wound care and dressing changes
  • Blood sugar testing and Diabetes Support
  • Fall Prevention and Safety Assessments
  • Skin Assessments for breakdown
  • Diabetic Foot Care
